Acne Scars

The word scar covers several different problems. Icepick scars are narrow, deep pits. Boxcar scars are wider with sharp edges. Rolling scars are shallow waves caused by fibrous bands pulling the skin down. Some marks are not indentations at all but flat red or brown stains left behind. Matching the type to the treatment family is the heart of a realistic plan.

Common causes

  • Deep inflammatory acne destroys collagen and leaves a depression when it heals
  • Picking and squeezing increase damage and the chance of scarring
  • Fibrous bands anchor skin to deeper tissue and create rolling shapes
  • Delayed treatment of cystic acne leaves more tissue loss
  • Genetic tendencies influence how skin heals

How doctors usually approach it

Icepick scars: targeted chemical or small-area methods
Narrow pits do not respond well to broad resurfacing. A focused acid technique applied into each scar, or small surgical approaches, is the category usually discussed.
Rolling scars: release plus stimulation
Where bands tether skin, a release technique called subcision lifts the depression. It is often followed by collagen-building injectables or microneedling to maintain the correction.
Boxcar scars: resurfacing lasers and microneedle RF
Ablative and non-ablative fractional lasers and microneedle radiofrequency remodel the shoulders and floor of wider scars over a course. Deeper settings give more change and more recovery.
Skin quality support
Picosecond lasers, polynucleotide boosters, PRP and PDLLA injectables are used to improve texture and softness around scars, with gradual and moderate effects.
What does not help
Surface facials, scrubs and light-only treatments will not lift a depression. Fillers can be used for select scars, but they are temporary and not a general answer.

Treatments often discussed

Resurfacing & ScarsMicroneedle RFMicroneedle RF is the middle option for scars, pores and lax skin: it works below the surface, so it changes texture with less visible injury than resurfacing lasers. From ₩400,000Resurfacing & ScarsCO2 Fractional LaserCO2 fractional laser is the strongest resurfacing option in the scar-and-texture group, reserved for visible scars and deeper lines where milder tools fall short. From ₩400,000Laser & PigmentPico LaserPico laser uses extremely short pulses, so it can be set for crisp spots, hazy tone or textured scars, and it is easiest to understand by which of those targets you have. From ₩220,000Skin BoostersRejuran (PN Skin Booster)A polynucleotide skin booster designed to support skin repair and quality, often discussed for thin, scarred or sensitive skin and delicate areas. From ₩300,000Resurfacing & ScarsErbium Fractional LaserErbium fractional laser resurfaces with less heat than CO2, so it is the mid-strength choice for texture and fine lines when you want shorter recovery.Price after consultationResurfacing & ScarsNon-Ablative Fractional Laser (Fraxel-type)Non-ablative fractional laser is the gentlest of the fractional lasers: the surface stays intact, so recovery is short, but change is slower and smaller.Price after consultationResurfacing & ScarsMicroneedlingMicroneedling is the mildest needle-based option for texture and shallow scars, and it is the baseline against which microneedle RF and lasers are compared.Price after consultationResurfacing & ScarsSubcision for Acne ScarsSubcision frees the fibrous bands that pull rolling acne scars downward, so it is the targeted option when the scar is a soft dip rather than a narrow pit.Price after consultationResurfacing & ScarsTCA CROSS for Icepick ScarsTCA CROSS is a small, targeted chemical technique for narrow, deep ice pick scars, the shape that broad resurfacing treatments tend to handle poorly.Price after consultationInjectablesJuvelook (PDLLA)A PDLLA stimulator given as shallow, small-volume injections, aimed at skin quality, shallow scars and fine lines more than at shaping.Price after consultationHair & ScalpPRP (Platelet-Rich Plasma) for Face and ScalpA concentrate of your own platelets is injected or applied to skin or scalp to support repair; evidence varies by use and results are gradual.Price after consultation

Matching scar type to the right treatment family

A simple test is how the scar changes when the skin is stretched. If it flattens, it is likely a rolling scar or a shallow boxcar, which tends to respond to release and resurfacing. If it stays sharp and deep when stretched, it is likely an icepick or deep boxcar, which responds better to targeted methods.

Most faces carry a mix. Doctors often combine categories in sequence, for example releasing tethered scars first, then using fractional laser or microneedle RF for texture, then maintaining with boosters.

Expect improvement, not erasure. Percent improvement is usually modest per session, and the course can span months. Active acne should be controlled first, since new inflammation makes new scars.

Acne Scars: common questions

Which scar treatment has the least downtime?

Microneedling, non-ablative lasers and some RF options generally involve shorter recovery than ablative resurfacing, but they typically need more sessions for a modest result.

Can I treat scars while I still have acne?

Usually the active acne is brought under control first. Your doctor may combine the stages in some cases.

Are results permanent?

Remodeled collagen can last, but aging and new acne continue to change the skin. Improvements are better viewed as long lasting rather than final.
