Subcision for Acne Scars

Not every acne scar is the same shape, and the shape decides the tool. Rolling scars, the soft wave-like depressions, are often held down by tissue beneath the skin. Subcision addresses that cause directly, which is why it is compared with resurfacing treatments that work from the surface.

How it works

After local anesthetic, a fine needle or blunt cannula is inserted just under the skin at the edge of the scar. The doctor sweeps it beneath the depression to cut the fibrous strands tethering the skin to deeper tissue. Releasing the skin lets it rise, and the small pocket that forms fills with blood and healing tissue, which can build collagen and support the new level. Sometimes a filler or other substance is placed to hold the space, but that is a separate decision. Improvement varies by person and scar type.

Who it may suit

  • Rolling acne scars with a soft, wave-like profile
  • Depressed scars that flatten when the skin is stretched
  • Scars that did not improve with surface treatments alone
  • Combining with laser or microneedle RF in a multi-step plan

The treatment day

  1. Identify tethered scars. The doctor stretches the skin and checks which scars lift, since only those suggest fibrous tethering.
  2. Mark and numb. The scars are marked, the skin is cleaned and local anesthetic is injected.
  3. Release the bands. The needle or cannula is moved under each scar to separate the tethering fibers.
  4. Press and protect. Pressure is applied to limit bruising, and a dressing or cold pack may be used.

Aftercare

  • Expect swelling and bruising for up to one to two weeks; cold packs help at first.
  • Avoid blood-thinning supplements and pain relievers unless the doctor says they are fine.
  • Keep puncture sites clean and avoid strenuous exercise, heat and massage for the period advised.
  • Contact the doctor for fever, a hot, growing swelling, pus, severe pain or lumps that worsen.

Things to know first

  • Bruising and swelling are expected; hematoma, infection, nodules, prolonged lumpiness and skin discoloration can occur.
  • Results can be uneven, and some scars may refill or need repeat sessions.
  • Bleeding disorders, blood thinners, active acne in the area, infection and keloid tendency require a doctor's review.
  • Pregnancy is generally a reason to postpone elective procedures.
  • It does not treat ice pick scars, flat pigment marks or loose skin.

How subcision compares with nearby options

The key difference is cause versus surface. Fractional lasers, microneedle RF and microneedling stimulate collagen across the skin, while subcision releases a specific mechanical problem under the scar. For rolling scars, this is why it is frequently the foundation, and surface treatments are layered on afterward to refine texture.

Against TCA CROSS, which is aimed at narrow, deep ice pick scars, subcision serves a different shape. A face often has several scar types, so combinations over time are common, with each step chosen for the profile it addresses. Fillers and collagen stimulators can add support to a released scar but do not release the tether.

Subcision is not helpful for flat marks, redness or pigment, and it does not lift generally loose skin. Because it is a minor procedure with bruising, it suits people who can accept more visible recovery than a typical laser session. A doctor can test which scars respond to stretching.

Subcision for Acne Scars: common questions

Which scars suit subcision?

Rolling scars that are tethered to deeper tissue. Narrow ice pick scars and flat marks usually need other approaches.

Is it done alone or with other treatments?

Often combined with lasers, microneedle RF or injectables over time, since faces commonly have more than one scar type.

How noticeable is the bruising?

Quite noticeable for some people, and it may take one to two weeks to fade. It varies by person and area.

How many sessions are needed?

Often one to a few, spaced several weeks apart, depending on the scars and response.
