Under-Eye Filler

Dark circles have several different causes, and a filler only suits one of them. This page helps separate a hollow caused by lost volume from pigment, puffiness or thin skin, and explains how tear trough filler compares with the other things people try.

How it works

A soft hyaluronic acid gel is placed along the hollow that starts at the inner eye corner and curves down onto the cheek, usually deep and close to the bone or in a layer beneath the muscle. The gel fills the depression, which reduces the shadow that falls across it and may make the area look smoother and less tired. It treats a shadow caused by shape. It does not change pigment, and it cannot tighten crepey skin. A needle or a blunt cannula is used, and very small amounts are placed in stages because the skin here is thin and shows irregularities easily.

Who it may suit

  • A clear groove or hollow under the eye that casts a shadow, especially in good lighting
  • Hollowing that has developed with age or that runs in the family
  • A tired look that is mainly due to shape rather than color
  • People with enough tissue thickness to hold a smooth layer of gel

The treatment day

  1. Light and shape assessment. The doctor checks whether the darkness is a shadow, pigment or both, often by changing the angle of light or gently stretching the skin.
  2. Numbing. Numbing cream is applied and left on before the procedure.
  3. Careful placement. A small amount of gel is placed with a fine needle or cannula, and the area is checked as treatment goes on.
  4. Review and cooling. Ice is applied and the doctor looks at symmetry with you sitting upright.

Aftercare

  • Use cold compresses gently in short intervals to limit swelling
  • Sleep with your head raised for the first night or two
  • Avoid rubbing the eye area, and skip intense exercise and saunas for a couple of days
  • Report any severe pain, visual change, or skin turning white or dusky right away

Things to know first

  • Swelling and bruising can last up to about a week or more in this thin-skinned area
  • Visible lumps, a bluish tint (the Tyndall effect) and persistent puffiness can occur, especially if too much gel is used or placed too shallowly
  • Vascular occlusion near the eye is rare but can threaten vision and needs immediate medical attention
  • Existing puffy eye bags, heavy fluid retention or very thin skin can make the area look worse after filler
  • Not advised in pregnancy, breastfeeding, active infection or with allergy to filler ingredients

How under-eye filler compares with nearby options

Start with what you actually see. A hollow with a shadow is the case for filler. Brown or gray pigment in the skin is better approached with topical care, light-based treatment or a regenerative booster. Bluish color from thin skin and visible vessels may improve slightly with a booster or skin-thickening approach, but filler can make the blue show more. Puffy bags usually need a different plan altogether, and filler can make them more obvious.

Compared with a polynucleotide or hyaluronic skin booster, filler changes shape immediately, while a booster works across thin skin over several sessions and gives modest changes in texture and quality. Some people have both a groove and thin skin and may be offered each in sequence. Collagen stimulators are generally not used in the lower eyelid area because of the risk of lumps.

If your concern is mostly color, or if you have prominent fat pads, ask the doctor candidly whether filler is likely to help. Declining a treatment that is not suited to you is a sign of careful assessment, not a limitation.

Under-Eye Filler: common questions

Will filler remove my dark circles?

Only if the darkness is a shadow from a hollow. It does not lighten pigment, and sometimes it does not improve circles caused by thin, bluish skin.

Why is this area considered higher risk?

The skin is thin, so lumps show, and blood vessels near the eye are connected to important structures. Placement should be done carefully by an experienced doctor.

Can it be dissolved if I do not like it?

Hyaluronic acid filler can usually be dissolved with an enzyme. A doctor should make that decision.

How long does it last?

Often for many months, and it varies by person and product. This area can hold filler longer than more mobile areas.
